Lipedema is a chronic condition seen almost exclusively in women, characterised by symmetrical, painful fat accumulation in the legs (and sometimes arms). Its defining feature: this fat is resistant to diet and exercise. Often mismanaged for years as a "weight problem", lipedema can be dramatically improved with the right diagnosis and lymph-sparing surgery.

Symptoms and Diagnosis

The typical picture: a slim upper body with symmetrical enlargement from the hips down to the ankles; legs that are tender to touch, bruise easily and feel heavy by the end of the day. The feet are typically spared — an important sign distinguishing lipedema from lymphoedema.

Diagnosis is clinical; staging (1-3) is based on skin surface and tissue structure. Onset or worsening around hormonal milestones — puberty, pregnancy, menopause — is characteristic.

Why Diet and Exercise Are Not Enough

Lipedema fat has a diseased connective-tissue structure distinct from normal storage fat. A calorie deficit strips fat from the rest of the body while largely sparing the lipedema areas — the source of the frustration so many patients describe.

Conservative care (compression garments, manual lymphatic drainage, exercise) manages symptoms but does not remove the tissue. Surgery is the only method that can permanently reduce it.

Lymph-Sparing Liposuction

Lipedema surgery is a specialised liposuction performed with tumescent technique and longitudinal cannula strokes parallel to the lymphatic vessels. The goal is maximal removal of diseased fat while protecting the lymphatic system.

The volume that can be removed safely in one session is limited; with extensive involvement, the legs are usually treated over 2-3 sessions, 6-12 weeks apart. The session plan is mapped at consultation according to stage and distribution.

Aftercare and Realistic Expectations

Compression garments are worn for 4-6 weeks; lymphatic drainage massage supports recovery. Most patients notice a clear reduction in pain, tenderness and easy bruising within the first months.

Surgery permanently reduces the disease burden in the treated areas — but lipedema is a chronic condition. Weight control and compression discipline remain part of the long-term outcome.
