The Three Components of Neck Ageing
First, fat: submental fat accumulation creates a double chin even in young patients. Second, muscle: the edges of the platysma separate with age, forming vertical bands. Third, skin: elasticity loss brings sagging and horizontal creasing.
In a young patient with good skin, the problem is usually fat alone — solved by chin liposuction. With advancing age, all three components are addressed together.
Technique Selection: Who Needs What
Fat excess alone → chin liposuction (tiny access points, performable even under local anaesthesia). Fat + platysmal bands → liposuction with platysmaplasty through a small submental incision (suturing the muscle edges at the midline). Significant skin excess → neck lift via peri-auricular incisions — at which point the operation frequently joins a lower facelift.
Examination decides the combination: skin recoil testing, band behaviour at rest and animation, and chin projection are read together.
Combining With a Facelift
The neck and lower face are parts of one anatomical unit; the SMAS layer and platysma are continuous. In significant laxity, lifting the neck alone can create a mismatch with the lower face — which is why advanced cases combine with a facelift for a more natural and durable result.
The decision is finding-based, not age-based: a 45-year-old with marked banding may benefit from the combination, while a 60-year-old with isolated fullness may need only liposuction.
Recovery
Chin liposuction: 2–3 days of social downtime, one week in a chin strap. Platysmaplasty: 5–7 days, with a night strap for 1–2 weeks. Neck lift: 10–14 days of social recovery, with most swelling and bruising resolving by two weeks; a sensation of tightness is normal for weeks.
